This is the tweet that alerted Bethesda of a pricing error. A MAJOR pricing error. Today Fallout 4 and its DLC were somehow free on the Xbox Store.
Earlier today Fallout 4 itself and expansions Automatron, Wasteland Workshop, and Far Harbor were all completely free. Bethesda has no idea how long the game was free for or how this huge error occurred.
Bethesda marketing VP Peter Hines was asked about the mistake on Twitter:

Microsoft has indeed rectified the issue, bring the game back to its actual price. A process of reomving Fallout 4 from the accounts of those who took advantage of the error is currently underway:
my understanding is that they're looking at who grabbed it while it was "free" and undoing that transaction. that's all

There is yet to be a statement from Microsoft on how the error occurred.
